Below is a list of local preferred medical services. If any Saint Martin's University Student-Athlete seeks outside medical care, an athletic trainer MUST be notified.  Proper paperwork must be brought back identifying the following:

            Reason for visit
            What images/tests were done and results
            What the medical provider found during evaluation
            Diagnosis
            Athletic/Class restrictions
            Follow up care
            Medical clearance or not for athletics?
            Signature/facility information
