Saint Martin's has moved to a paperless medical eligibility process. Every athlete will be required to sign up for a login and complete all required information before they will be cleared for participation. Below is a link to a letter explaining how to create a login. An email will be sent to all athletes on record with the coach that will provide a school ID that must be entered in order to create a login.
Medical Eligibility paperwork must be done by August 1st!

ADD/ADHD Medication
Any athlete taking an ADD/ADHD Medication must have the following form completed by the prescribing physician prior to each year of athletics. If there is a question regarding a medication, please contact your prescribing physician or the Head Athletic Trainer.
